Maryland MOLST is a portable and enduring medical order signed by a physician or nurse practitioner that contains orders about CPR and other life-sustaining treatments. MOLST helps to ensure that a patient’s wishes to receive or decline care are honored throughout the health care system.
Effective October 1, 2011, MOLST must be completed for all individuals admitted to nursing homes, assisted living facilities, hospices, home health agencies, and dialysis centers and for certain individuals admitted to hospitals. It should also be ocmpleted for anyone who wants limitations on CPR in any setting and for any patient that requests a MOLST form.
Completion of the MOLST form is based on an informed discussion between a health care professionals and patient or authorized decision maker. It may also be based on a patient’s advance directive. Even if the practitioner who signs the MOLST form is not aon a facility’s medical staff, the MOLST form is valid. The orders on the Maryland MOLST form must be honored by EMS personnel and all health care providers, facilities, and programs across Maryland.
