Objectives:

Health care decisions are difficult to make and many providers struggle with their ability to comply with resident and family wishes, especially when there are differences. Attend this program to understand your obligation under certain circum­stances and identify potential deficiencies for noncompliance.

  • Familiarity with legal framework in Maryland

  • Ability to discuss effect of living will on authority of proxy

  • Understand provider obligations in facilitating health care decision making

  • Identify deficiencies related to advanced health care decision planning

Nurses: This continuing nursing education activity was approved by the Maryland Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation, for three (3) contact hours.

Administrators: This educational offering has been reviewed by the National Continuing Education Review Service (NCERS) of the National Association of Boards of Examiners of Long Term Care Administrators (NAB) and approved for 3 clock hours and 3 participant hours.

Social Workers: HFAM is an approved sponsor by the Board of Social Work Examiners. This program is approved for 3 Social Work Category I Education units.

Directions:

HFAM Meeting Room:

From Baltimore and North. Take I-95 South toward Washington, DC. Take Route 175 West towards Colum­bia. ­After the Gateway Center Exit get in right hand lane. Take the exit for Snowden River Parkway South. Make a left at the bottom of the exit ramp onto Snowden River Parkway. Pro­ceed on Snowden River Pkwy. At the 6th light turn right onto Minstrel Way. Follow Minstrel Way to your left; take the last driveway on left.

From Washington DC and South. Take I-95 North towards Baltimore. Take Route 32 West towards Colum­bia. Take the Broken Land Park­way exit North. Turn slight right onto Snowden River Parkway. At the first traffic light turn left onto Minstrel Way. Follow Minstrel Way to your left; take the last driveway on left.

From Annapolis and East. Take Route 50 West to I-97 North towards Baltimore. Take Route 32 West to­wards MD-3 South/Colum­bia/Bowie. Straight on Route 32 for approximately 16 miles. Take the Broken Land Parkway exit North. Turn slight right onto Snowden River Parkway. At the first traffic light turn left onto Minstrel Way. Follow Minstrel Way to your left; take the last driveway on left.

From Western Maryland. Take Route 70 East to Route 29 South exit, towards Columbia/Washington. Take Route 100 East exit, on the left towards Glen Burnie. Take the Snow­den River Parkway exit. Proceed on Snowden River Parkway. At the 8th light turn right onto Minstrel Way. Follow Minstrel Way to your left; take the last driveway on left.

Charlotte Hall Veterans Home

From Baltimore and North. Take I-97 South to Route 3 South, merge into Route 301 South towards Waldorf. In Waldorf, make a left at Route 5 By-pass South towards Leonardtown/ St. Mary’s City. Proceed to Charlotte Hall, MD. Turn right onto Charlotte hall School Road. After Stop sign, Charlotte Hall Veterans Home is straight ahead.

From Western MD & Washington, DC. Take I-70 East to I- 270 South to Washington, DC loop, I-495 East. Follow loop East (I-495/I-95) to Route 5 South towards Waldorf/Leonardtown/St. Mary’s City. At Route 301 in Waldorf, follow Route 5 By-pass. Proceed to Charlotte Hall, MD. Turn right onto Charlotte hall School Road. After Stop sign, Charlotte Hall Veterans Home is straight ahead.

From the Eastern Shore. Take Route 50 West past Annapolis to Route 301 South towards Waldorf. In Waldorf, make a left at Route 5 By-pass South towards Leonardtown/St. Mary’s City. Proceed to Charlotte Hall, MD. Turn right onto Charlotte hall School Road. After Stop sign, Charlotte Hall Veterans Home is straight ahead.
